Registration, consent and safeguarding form for all Women & Girls (U18) players before taking part in Federation activities. It covers player and parent/guardian details, medical and emergency authorisation, travel and supervision consent, image rights, and the FCC No-Objection Certificate (NOC) rule. It must be completed, signed and submitted to the club.
